Continuing care services, including long-term care facilities, are provided by the Department of Health's Single Entry Access (SEA) system. Call 1-800-225-7225 to speak with a care coordinator who will help you identify your needs (or your family member's needs) for care and the services required, including placement in a long-term care facility. Complete financial and medical information will be collected and placed on a waiting list for the facilities of your choice.

If you are considering Saint Vincent's as an option for care, we suggest you arrange for a tour of our facility with the Social Worker. This is helpful to familiarize yourself with the services we offer and to deal with any questions you may have.
